Alex has been caught shoplifting; Abby and Luka are interviewing new nannies, including "trailer park woman" (hey -- someone Abby can identify with!), a "smoking hot" Brazilian woman, and a stout, dour Eastern European woman.

There's some soapboxing about people using the ER for primary care when an uninsured man needs a prescription for his son; and there are a pair of recurring roles for a young woman awaiting a liver transplant, and her mother.

Joe's nanny flaked.

Abby and Luka had to bring him to ER.

Abby apologizes because Luka had to change the baby in the break room and it doesn't smells nice.

Willis' the big patient this week.

He's a royal pain in the ass and very offensive.

He thinks he has flu.

He's rude to the doctors and insults Sam.

Turns out that he was an abused kid, the marks are still all over his body. Sad! Very sad, sweethearts!

He doesn't have the flu, but a heart twice its normal size.

Willis leaves the hospital, he doesn't want to be treated.

Hours late he comes back bruised and battered.

He doesn't remember being there, and he seems like a totally different man.

And you want to know why, kids?

Because he has mutiple personalities.

Alex got caught stealing beer.

He calls Sam a looser.

She's pissed and tells him to do his homework.

Neela thinks Gates' an ass.

She tells him that.

Morris and Sam treat Willis/Clyde/Andre, the three personalities of that poor kid.

Morris and Sam make a great team!

Morris' the one who finds out what's wrong. He thinks nobody respects him and that people see him as a joke.

Sam tells Morris that he isn't a joke, he's an attending.

Flavia's the superhot Brazilian applying to the nanny slot.

So hot everybody stares at her.

I don't think she gets the job, do you?

Mary's a perfect midwestern applying to the nanny slot. She says she's so good that by the third week the kids are calling her mommy.

I don't she gets the job either, do you?

Willis Payton arrives at the overcrowded ER triage area while a homeless man rambles about how he can smell them coming but we can't.

After seeing a burned patient come in screaming, Willis’ demeanor changes dramatically, and he demands treatment at the window. Sam seems to calm him down and get him to fill out the forms.

Pratt’s barbershop friend shows up and then goes away as Abby announces that the break room now smells … Luka changed baby Joe’s diaper in there. After personal concerns are expressed by Pratt, Frank and Morris, we learn the baby sitter didn’t show up and they had to bring the baby into the ER.

It seems that Abby and Luka are interviewing possible baby sitters, including an “exotic” woman, a woman who wants to smoke in the hospital, and someone who at first seems good, but who believes the baby will bond with her. No clue as to who gets the job. Only later, probably Act II, Luka is holding baby Joe outside a lecture hall and the tension is increasing. Inside, Abby is debating some medical point with Crenshaw.

Meanwhile, Willis has an enlarged heart and the docs want to drain the fluid. Willis refuses and leaves. But hours later, a man who looks the same with the same symptoms shows up, and it is Morris who eventually realizes that Willis has multiple personalities. And it is Sam who solves the situation by acknowledging that he scared and hurt, but that he is safe in the ER. And then she demands to speak to an alternate identity and bullies that personality into accepting treatment. In fact, there are three personalities, and Morris ends the show talking with the “little boy” personality and talking about Hot Wheel cars.

All the while, in contrast to the mystery Willis provides are two different child patients. Mikela is waiting for a transplant but her recent illness does not move her up the list. And then there is a boy Luka is injecting with something, and the sight of the needle sends Willis into another personality.

Sam has Alex at the ER and she’s been informed that he was stealing beer, but he’s unrepentant, and ordered to the break room to do his homework. And Gates somehow angers Neela who doesn’t think much of him. Ray’s here too – not quite believing Morris’ diagnosis of Willis.

In the teaser, Luka arrives at the ER (perhaps with baby Joe in tow), noting that he training the baby so he can “retire early.”

Pratt accepts from the EMT’s a young girl with possible hepato-renal syndrome. Her family comes in as well. Turns out that the young girl was dehydrated and improves later, which means she is not a higher status for a liver transplant. Her mother is quite distressed seeing her daughter suffer this way.

The other man who fought with Willis has a possible broken nose.

Kerry and Hope treat a man who needs his prescription refilled but the family clinic cannot schedule him for an appointment for a few months. Kerry insists the man go to the clinic and she will see that he gets in quickly. But when the appointment at the family clinic is still a month away, Hope somehow gets the man a month’s worth of medicine.

Sam is called to the Jumbo Mart where the manager has apprehended Alex stealing beer. Alex is unrepentant, and ordered to the break room to do his homework.

Gates and Neela were in the discussion of the young girl’s status for a transplant and leave together. They talk about an upcoming M&M and who is nervous. Again, Ray separates their conversation by assigning Gates a patient, and then Ray talks to Neela, and longingly watches her walk away. Later, after the M&M, Neela is quite angry at Gates.

After is seems Abby has left work with baby Joe, she’s in the park with Ames (Luka’s patient who sued and lost) having a hot dog and talking about Ames’ children. Ames notes he’s not seen Abby there before, but that he comes to the park a lot with his children.

09/16 - Episode 13.?? - Jigsaw: A 19-year-old girl named Mikela is waiting for a liver transplant. Her family has mixed feelings wanting her condition to worsen so she'll move up the waiting list but also wanting her to get better. Mikela and her family may recur in other episodes. A man comes to the ER with his son to get a prescription refilled. After a fight on a bus, two men are brought in the ER. Luka and Abby are looking for a nanny. They will interview various women all of diffirent backgrounds and ethnicities. Alex gets caught shoplifting at the Jumbo Mart. Dubenko, Ray, Malik, Crenshaw, Gates, Neela, Marquez, Sam, Morris, and Pratt, also appear.

They are going the creepy, stalker route. As Luka, Abby, and Joe leave the hospital, Ames watches them from inside the Jumbo Mart. It's noted that this "is definitely not over." Later, as Luka and Abby sit down to dinner, Abby comments that she cannot find Joe's frog and that it was difficult to get him down without it. The next shot is of Ames in his car (where he's living) with Baby Joe's stuffed frog.

"Jigsaw"
A PUZZLING PATIENT COMES INTO THE ER AND LEAVES THE DOCTORS STUNNED-- FOREST WHITAKER GUEST STARS- Abby (Maura Tierney) and Kovac (Goran Visnjic) finally decide to hire a nanny to care for their son Joe, but are troubled when they meet the potential candidates. Morris (Scott Grimes) and Sam (Linda Cardellini) try to treat a biligerant patient that comes in with seemingly simple flu. However, when they discover that his condition is much more complicated, the patient refuses treatment. During a trip to the park with Joe, Abby inadvertantly meets Curtis Ames (guest star Forest Whitaker). Meanwhile, Gates (John Stamos) and Neela (Parminder Nagra) argue with each other at a hospital board meeting. Laura Innes, Mekhi Phifer and Shane West also star.
