A round of vodka and tonics for everyone who not only guessed Pratt is going to help out the noble Dr. Carter in the camp in Darfur, but that he acts a little smug and know it all about it to the first attractive lady he meets on the plane over. Her name is Fatima and she works for the Ministry of Education and is coming back from a UN conference. Our Man Greg thinks she'll actually be impressed that he's a doctor and tells her he's going over for two weeks, then he's going to go hit the beaches in Kenya.

Silly boy actually brags that he can handle anything he'll see in the Sudan because he works in a Chicago ER. Smart girl, she's not impressed and lays out for him just how bad things get. Then tells him she hopes he enjoys the beaches. Fatima is my kind of girl, darlings.

I received some more on this. The earlier "Pratt Goes to Africa" info and these newer roles are specifically mentioned as being episode 19.

Other plot threads:
A twelve year old girl and her grandmother are treated. They are each other's sole caregiver, and the grandmother is discovered to have dementia.

Weaver is undergoing physical therapy after her surgery.

A sherriff brings Sam's ex, Steve, into the ER for treatment.

The jail doctor was worried that Sam's ex has appendicitis, so he is sent to County for a surgical consult and convenient reunion with Luka. Oh my.

Morris took the dream job with the pharmaceutical company to start July 1st and he's already spending his signing bonus. Silly boy, doesn't he know this things are never that easy in TV land?

My goodness the living arrangements are getting complicated darlings. Neela is staying at Abby's place while Abby is pretending she hasn't really moved in with Luka.

Dr. Morris is showing off his new Armani suit and his new patient thinks he's just the finest looking doctor she's ever seen. She's bipolar, manic and took a bad fall, and Morris argues with Albright over whether she needs surgery. Guess what darlings? Morris makes a brilliant diagnosis and makes Albright look so, so bad, everyone is impressed except her.

Oh dear, there's a 12 year old who's very smart and when her grandmother comes into the ER to get her after an accident where the girl tried to drive the car, Abby realizes Gandma has multi-infarct dementia and the girl is trying to take care of her alone. We all know how Abby deals with this, don't we? She complains and makes excuses and argues with Luka when he tells her to call social services, of course.

Oh goodness. The Sherrif thought Steve was faking the appendix to get out of something but he wasn't. Unless he's figured out how to fake a burst appendix really, really well. Albright offers to let Neela scrub in on the surgery, even though she's finished her surgical elective.

Dr. Clemente is a mess, darlings. He asks Abby for Ambien and she gives him a scrip for only two pills, tells him to get any more from his regular doctor. He grabs someone who was following him out of the hospital cause he thinks it was Jodie's husband, but it was only a patient who wanted to ask him more questions. I don't think this is going to go anywhere good, do you?

Weaver 'graduates' from her post op physical therapy and has done pretty well apparently. She seems a bit apprehensive about returing to work, though she's been keeping up with paperwork from home. She still has a crutch, the therapist seems to indicate that she either doesn't need it anymore or won't need it much longer.

Abby is upset about 12 year old who was in a MVC while driving. Her parents are dead, she lives with her grandmother and a elderly dog and is apparently taking care of them rather than vice versa. Grandma doesn't seem to know who the president is nor how much it costs to mail a letter, she thinks it's 19 cents. Not sure what day it is either. They work her up and she has dementia. Ruthie needs to be placed in a foster home. Abby, of course, is reticent, asking Luka if they just couldn't stay as they are. She actually doesn't call them until Luka forces the issue. He tells her she has a responsibility to the girl and that social services needs to be involved. The girl is very upset, Abby tells her they will find a foster home that will let her bring her dog (good luck with that, Abby!) and the grandmother is very reassuring to the girl, reminding her that, when her parents died, it was the worst thing possible, but they have had so many wonderful times in the years since then and, just because foster care sounds terrible, it doesn't mean that they won't have good times ahead of them, too.

Steve's appendix has ruptured and Albright offers to let Neela scrub in even though she is done with her surgical elective. Steve tells Sam he wasn't faking, sounds like she might've suspected that when he was brought in.

Morris' patient fell 10 feet out a window and Albright diagnoses blood in her belly, but it turns out it is fluid from kidney problems caused by long term lithium use for BPD. He stops Albright from taking her to the OR just in the nick of time. The patient, RainyDay, is quite manic and seems to be fond of Morris.

Her remarks to Neela about not actully living with Luka, despite the fact that she is very pregnant with his kid and never sleeps at her own place are just weird, too. From what Neela says about Abby's alarm clock, I'd guess she is sleeping in Abby's bed. Abby turns to Luka to confirm to Neela that they are NOT living together, but a trauma arrives and Neela has to rush off before he can answer.

At the end of the eppy, Abby's patient, Ruthie, shows her that she has now acquired a bunch of bus passes so she won't have to drive anymore, she also has gone out and gotten a bunch of books on caring for people with dementia at home as well as beginner cook books, etc. My guess is that she is going to try to convince Abby to let her escape from the ER without seeing social services. She went and did these things by claiming the dog needed to 'take a dump' and apparently missed the social worker when she came down. No clue as to what Abby does, but I have a pretty good idea that, as per usual, Abby will decide that the 12 year old is perfectly capable of fending for herself. After all, she took out a couple of library books and paid for some bus passes, she is clearly capable of handling her demented grandma.

Ironically, the eppy starts with Clemente giving a guy with a migraine a script for only two vicodin for his migraine, teling him to see his family doc for more, suspecting he might be drug seeking and ends with Abby ( ....) giving Clemente only two Ambien, telling him to get a family doc if he needs more.

It appears that Neela and Clemente do the initial assessment of Steve and release him to return to jail. Clemente refers to Steve as 'The Birdman of Alcatraz'....

These spoilers cover just the first two acts plus a little bit of the beginning of Act III, so I’m thinking that the 2/3rds are is about County, and the last bit of Act III and Act IV, give or take some, is about Darfur.

Abby treats a grandmother and granddaughter who parents were killed when she was a baby. The grandmother is diagnosed with multi-farct dementia – she is going to get worse. Abby is reluctant to call social services, which will split up the family. And despite Luka’s urging that she do so, the 12 year old girl’s desire to take care of her grandmother is going to make Abby’s decision tough.

Meanwhile, we learn that Neela has moved out of Ray’s apartment and is staying at Abby’s place. However, Abby insists to Neela she has not moved in with Luka, even asking Luka to confirm that fact to Neela. We don’t really know his answer.

Morris has a new, expensive suit he purchased with his signing bonus with the pharmaceutical company, although he has not started work yet. He spills coffee on it and has to change to scrubs. But his patient is diagnosed quickly by Albright as needing surgery although Morris thinks not. Morris embarrasses Albright with his accurate diagnosis.

Kerry finishes her physiotherapy and no longer has to walk with a cane. She contemplates life without her “crutch.”

And Clemente has been away, in New Jersey. Josie is doing fine but he’s not sleeping well. He asks Abby for a prescription for sleeping pills and she does – for 2 pills, telling him to find a personal physician.

Steve is brought to the ER with a burst appendix. I’m not quite sure what happens but it seems Sam arranges things so Steve can get quickly into an OR for surgery. I have a feeling the sheriff who brought Steve in thought he was faking and Sam stood up for Steve.

And we have the earlier spoilers about Pratt flying to Darfur and trying to impress a fellow female traveler who impresses him with the tragedy, the paralysis, of the situation in Darfur.

It strikes me that this episode, based on the title’s implications, is about people having to face something about themselves that they don’t want to admit (good or bad), or being forced to accept things they are reluctant to accept. I’m unsure what the outcome is regards to these characters in Chicago, or as Pratt travels to the Darfur region to help Carter.

02/22 - Episode 12.19 - No Place to Hide: An on-edge Clemente takes care of 45-year-old and mild-mannered man named Ottley who complains when it's Clemente and not Pratt who deals with him. After dealing with the patient, Clemente asks Jerry if there are anymore patients. It is then, that Clemente is reminded his shift ended two hours prior. As he walks home, Clemente notices someone is following him. He hides, pops up behind the man, and shoves him back to realize that it's Ottley... Neela moved in Abby's apartment. Abby wonders if she officially moved in with Kovac or not. A 43-year-old named RainyDay is brought in after she fell from a second story window while cleaning it. Morris announces that he got a job at a pharmaceuticals company and will start working there on July 1st. Will he work both there and at the hospital? Abby takes care of a 12-year-old named Ruthie who complains of tenderness over her sternum. Ruthie's farting dog came along for the ride. Her grandmother, who is her legal guardian, comes to the ER to check on Ruthie. Abby checks the grandmother's cough as she has been coughing for a few years now. As she chats with the her, Abby comes to the conclusion that she is not fit to take care of Ruthie. Kovac must deal with a inmate who is brought in when the jail's doctor suspects appendicitis. The inmate? Steve Curtis, Sam's ex! Weaver has her last physical therapy session. Sam, Frank, Albright, Dumar, Ray, Haleh, Marquez, Pickman, and Jane, also appear.

WISE BEYOND HER YEARS- NOAH WYLE AND DIANE LADD GUEST STAR-- Abby (Maura Tierney) senses that something is not quite right when a grandmother (Guest star Diane Ladd) and her 12 year-old grandaughter are brought into the ER. Sam's (Linda Cardellini) imprisoned ex-husband Steve (Garret Dillahunt) is brought into the ER and all the doctors assume that he is faking, but when further tests prove otherwise, Steve finds his way to the operating table. Meanwhile, tensions run high when Morris (Scott Grimes) and Albright (Dahlia Salem) argue over a patient, but when Morris denies a surgery, Albright quickly changes her tune. Meanwhile, Pratt (Mekhi Phifer) heads to Africa to help Carter (Noah Wyle) and quickly realizes how difficult life will be. Laura Innes, Goran Visnjic, Parminder Nagra and Shane West also star. This eppy airs 4/27/06

Diane Ladd lends some grace and dignity to the role of a grandmother who attracts Abby’s concern when she and her grandmother are admitted to the ER. Also Garret Dillahunt appears as Sam’s incarcerated ex-husband, whose health complaints turn out to have merit.
